The above email template can be used to send to potential mentors. Make sure you copy and paste the template and input all necessary information that is specific to you.
The above mentor letter can be sent to mentors considering the program and wanting more information on expectations, etc. This form reflects the correct dates for the 2018-2019 school year.
The link above will take your mentor straight to the required forms for the background check. Copy and paste the link in an email and send it to your mentor. Doing this means I only need the name of your mentor on the "Student Contact List" (Google Sheets file I shared with you). At a later date you, you will complete form #1 which you will receive from me once second semester begins.
The above file is the COMPLETE set of all 3 required forms that mentors must complete. The file includes the directions page as well which details each form and why we need all the information listed. Please make sure mentors read the directions for each form. THESE FORMS ARE DUE ON DAY #1 OF CLASS - Wednesday, August 22nd, 2018.
